HEART FAILURE CARE

Helping you manage heart failure

Despite its name, “heart failure” doesn’t mean the heart has stopped—it means it’s not pumping as efficiently as it should. At Grady, we bring expert care and innovative support programs to help you manage your condition and live a full, active life.

OVERVIEW

Innovative care for a healthier heart

At the Grady Heart & Vascular Center, we combine cutting-edge treatments with real-world support to help heart failure patients stay healthier and out of the hospital.

  • Since 2011, our approach has transformed heart failure treatment, tailoring solutions to each patient’s unique needs.
  • Our initiatives have cut hospital readmission rates from 25% to less than 11%.
  • We help patients overcome barriers like cost and transportation by providing financial aid, rideshare assistance, and home visits for at-risk patients.
  • Telehealth technology tracks high-risk patients’ blood pressure and weight to catch problems early.
  • We’ve earned the American Heart Association’s Heart Failure Gold Plus Quality Achievement Award and a Gage Award for innovation, recognizing our commitment to top-tier heart failure care.

Are referrals needed?

Yes. Grady’s Heart & Vascular Center accepts new patients with referrals from either primary care providers or other specialists. Please:

  • Ask your provider to use our patient referral checklist and email us the details. We will contact you to schedule an appointment.
  • If you are an existing Grady patient, visit MyChart or call our scheduling line at (404) 616-1000 to make an appointment.

CONDITIONS TREATED

What conditions do we treat?

Heart failure can affect the left or right side of the heart, often caused by coronary artery disease, heart attacks, high blood pressure, or lung issues. There are two main types:

Systolic heart failure occurs when the left ventricle can’t contract fully, impairing blood flow. It’s usually treated with medication, devices, or sometimes surgery.

Diastolic heart failure happens when the left ventricle becomes stiff and can’t relax properly, often affecting older women with other heart or lung conditions. Treatment is similar to systolic heart failure.
