How to Make an Appointment

Thanks for choosing Grady. We can’t wait to see you. There are a couple of different ways to get an appointment.

By Phone

Call our 24/7 scheduling center at (404) 616-1000, and one of our scheduling representatives can work with you to connect you to the correct provider and service. Additionally, they will work with you to determine if a referral is required for your visit.

MyChart

If you are an existing Grady patient, have a MyChart account, and want to avoid waiting on hold, you can log in here and request an appointment without having to pick up the phone.

If this is a medical emergency, please dial 911 immediately, and ask to be taken to Grady.

What to Bring

Plan to arrive about 30 minutes before your appointment so you have time to fill out any necessary forms or ask any questions you may have.

Here are some things you’ll want to bring to your appointment:

  • Your photo ID
  • Your health insurance plan card
  • Your prescription insurance plan card
  • Referral or authorization forms
  • All current medications in their original containers
  • Any outside medical records, X-rays, or prior test results not taken at this hospital (if your physician has requested them)
  • Any questions you have for your doctor
  • Follow ALL directions regarding food and drink before your appointment or your test may be rescheduled

Grady accepts private health insurance, Medicare, Medicaid and workers’ compensation. We accept cash, checks, traveler’s check, money orders, Discover, Visa, MasterCard and American Express for payment. For detailed information, visit Billing and Insurance.

Referrals

Radiology appointments and most of our specialty clinics do require a referral. A referral is a request or order by your doctor to have an exam, test, or procedure performed at Grady.

If you don’t have a referral but need care, we can help. Call us at (404) 616-1000, and we will help set you up with an appointment in one of Grady’s Primary Care Centers, where a physician can partner with you and refer you to the appropriate specialty clinic.

Or, ask your provider to complete the Grady Referral Request Form and fax it to us at (404) 489-6103. We will contact you to schedule an appointment.

If you already have a referral, please call (404) 616-1000 to schedule your appointment or to check on the status of an existing referral.
