Grady’s Health Information Management department is responsible for keeping complete medical records for each patient.
Patients have the right to receive copies of their medical records. By completing our medical records consent form, we can send your medical records to a physician, another hospital or provide them to you.
Prior to receiving the copies, our medical records consent form must be completed and proof of identification provided. A fee may be assessed for locating and copying your record; you will be notified if any fees will be incurred.

Complete the medical records consent form on-site in the Health Information Management Department at Grady. Health Information Management (Medical Records) is located on the first floor of Grady Memorial Hospital – Room E138.
