From routine health concerns, to emotional health, to more serious matters, our physicians work with other Grady specialists to provide you with the very best care – often right in your neighborhood. Whether you need routine care or help with joint pain or controlling your blood pressure or blood sugar, we’re here for you.
Most importantly, our staff specializes in meeting each patient’s unique needs. Our goal is to keep people as healthy as possible for as long as possible.
Why Choose Us
Nobody in Atlanta offers better, more conveniently located care. Our doctors work with older people who may be experiencing a variety of health concerns related to aging. Most visits can be handled in Grady’s six neighborhood health centers, which are staffed and equipped to perform comprehensive evaluations and to recommend individual treatment plans that addresses each patient’s medical, functional, and emotional needs.
Our neighborhood health centers are able to provide all the care you need for annual wellness visits and checkups, which are designed to:
- Identify your health issues
- Develop a tailored treatment plan for you
Grady’s neighborhood health centers are also the gateway to all the specialties and expertise you may need from Grady Health System, which offers the most comprehensive care available in metro Atlanta.
We provide behavioral health counseling services for older patients facing mental health issues, including anxiety and depression.
Our gerontology specialists include nurses, social workers, physical and occupational therapists, and speech pathologists and audiologists, who work together to help you manage age-related disabilities and illnesses. We also provide access to all Grady specialties necessary to keep you healthy or restore your health.
Grady is a founding member of Georgia Memory Net, a statewide program dedicated to the treatment of memory problems and the early diagnosis of Alzheimer’s disease. The clinic conducts evaluations and makes referrals to primary care physicians who can provide appropriate ongoing care.
We provide specialized geriatric consultations for frail older adults with complex, life-threatening, or chronic conditions, including dementia, delirium, and persistent pain. Our program draws on resources within Grady to enhance hospital care and help to smooth the transition to subsequent care at home or in other care settings.
The center provides care for patients who are at least 75 years old and experiencing memory loss.
In addition to getting regular physicals and screenings, every senior should take the time to schedule an annual wellness visit with their primary care provider.
Medicare covers this yearly appointment to give you the opportunity to collaborate with your providers on a preventive care plan for the coming year. This isn’t the same thing as a head-to-toe physical exam, but the annual wellness visit is an important part of senior health care.
When you become a patient at Grady, you have access to on-site services like our diagnostic labs, pharmacies, rehabilitation and therapy, and imaging, which are available at Grady and its neighborhood health centers. You also have access to all of Grady’s total wellness classes aimed at helping you to quit smoking, eat healthier, and live longer. And, we provide emotional support if you are dealing with addiction, anxiety, and depression.
Grady offers seniors discounts on parking and over-the-counter medicines from our pharmacy if you are age 60 or over.
