ED Locker Request
We reserved lockers for staff whose primary unit is the Emergency Department. Use this form to request a locker or verify your assignment.
To verify your locker assignment, please put the number of the locker you currently occupy and your locker partner’s name.
To request a locker, please know all lockers are shared. If you have a locker partner in mind, place their name in the form. Otherwise, a partner will be assigned.
